MicroRNAs (miRNAs) are a class of small non-coding RNA molecules of approximately 22 nucleotides that regulate gene expression at the post-transcriptional level. Alterations in miRNA expression patterns correlate with a wide spectrum of pathological conditions, including cancer. miRNA profiling was mostly performed, in solid tissues, obtained by invasive diagnostic procedures. However, miRNAs in biofluids, such as serum and plasma, show high stability resulting from the formation of complexes with specific protein or incorporation within circulating exosomes or other microvesicles. Circulating miRNAs could be reliable biomarkers for early-stage cancer diagnosis, prognosis and response to therapy. In this chapter, we analyze the major pre-analytical and analytical challenges in experimental design for circulating miRNA detection, focusing on exosome fraction and microarray-based approach.

The focus of this chapter is on the development of miRNA diagnostics and the process for miRNA biomarker development. We discuss pancreatic cancer as a model for the application of miRNA-based tests in different clinical biopsy specimen types. Asuragen’s miRInform® Pancreas test illustrates the development of a laboratory developed test (LDT) that was validated in compliance with Clinical Laboratory Improvement Amendments (CLIA) and College of American Pathologists (CAP) regulations. We provide an overview of work on miRNA-based signatures which is focused on resolving clinical dilemmas in solid and cystic pancreatic lesions. We also review some of the promising miRNA candidates identified in biofluids from other cancer types. These studies highlight the emergence of miRNA-based diagnostic tools which have the potential to impact the clinical management of cancer by guiding early diagnosis, aiding in the assessment of patients’ prognosis, and enabling the identification of novel, more effective therapeutic targets.

Short non-coding RNA molecules, microRNAs (miRNAs), post-transcriptionally regulate gene expression in living cells. In recent years, miRNAs have been found in a wide spectrum of mammalian body fluids including blood plasma, saliva, urine, milk, seminal plasma, tears and amniotic fluid as extracellular circulating nuclease-resistant entities. The changes in miRNA spectra observed in certain fluids correlated with various pathological conditions suggesting that extracellular miRNAs can serve as informative biomarkers for certain diseases including cancer. However, the mechanism of generation and a biological role of extracellular miRNAs remain unclear. The current theories regarding extracellular miRNA origin and function suggest that these miRNAs can be either non-specific ‘by-products’ of cellular activity and cell death or specifically released cell-cell signaling messengers. The goal of this Research Topic is to bring together up-to-date knowledge about the extracellular miRNA and its role in disease diagnostics and, possibly, inter-cellular communication.
