En la primera de ellas, se realiza una revisión desde el punto de vista teórico del concepto de salud y sistema sanitario ya que su definición ha ido evolucionando y procurando adecuarse al contexto de cambios sociales, políticos y culturales de la sociedad. Para ello, se han profundizado en los objetivos que deben presentar los sistemas a la hora de proveer servicios sanitarios. Además, se presenta una descripción de las características de los sistemas sanitarios de los países de la OCDE siguiendo dos grandes patrones; Sistemas de Seguridad Social financiados por cotizaciones sociales obligatorias y otro basado en Sistemas Nacionales de Salud financiados a través de impuestos. En base a los conceptos anteriores, en la segunda parte, se presenta un análisis de la estructura organizativa del Sistema Nacional de Salud en España en la que se especifica el reparto de competencias sanitarias asumidas por el Estado, las Comunidades Autónomas y las Corporaciones Locales. Seguidamente, se realiza una revisión histórica del proceso de descentralización para comprender el origen del SNS. Finalmente, se continúa con el repaso de los aspectos del Sistema Nacional de Salud referidos a la financiación, la evolución del gasto y la población cubierta. Between 1977 and 1986, Spain underwent a comprehensive tax reform which shaped its fiscal system until today. It was made in connection with the transition to democracy and indeed was understood as a fundamental part of the political change. The book situates the reform both within Spanish history and international trends in tax systems and connects it to the expansion of the welfare state and regional decentralization in Spain. The analysis reveals that the tax system failed to attain progressivity, and significant levels of fraud had a noticeable impact on inequality. Because of this, fiscal redistribution remained limited. In the new political economy of the second globalization, late democratic and fiscal transitioners were unable to emulate the path of the welfare state forerunners.

Following three introductory, stage-setting chapters, the handbook offers country cases divided into seven regional sections, each of which begins with a short regional outlook chapter that highlights the region's common characteristics and divergent paths taken by the separate countries, including comparative data on health system financing, healthcare access, and the political salience of health. Each regional section contains at least one detailed main case, followed by shorter treatments of the other countries in the region. Country chapters feature a historical overview focusing on the country's progression through a series of political regimes and the consequences of this history for the health system; an overview of the institutions and functioning of the contemporary health system; and a political narrative tracing the politics of health policy since 1989.
