Physical activity is crucial for health maintenance of individuals with spinal cord injury (SCI), yet 50% of this population lead sedentary lives. Several modifiable challenges to physical activity behavior exist including inaccessible environments, negative body-self relationship, minimized exercise identity and knowledge. Digital health technologies, such as internet-based tele services, offer a potential platform to address these obstacles, but SCI-specific methods and evidence are needed. We developed an 8-week synchronous group tele-exercise program for individuals with SCI grounded by Social Cognitive Theory (SCT) to address societal barriers to exercise to influence individual determinants of physical activity behavior. Our feasibility study with 11 participants demonstrated 100% retention, high class attendance, and participant reports of increased exercise knowledge, confidence, and motivation. Given strong feasibility, the next step to evaluate the impact of this program was to assess the effects of group tele-exercise participation on physical activity determinants and behavior as identified by SCT in individuals with SCI. This clinically registered non-randomized trial [NCT05360719] used convergent mixed methodology. The single-group repeated measures design allowed for quantitative and qualitative assessment of primary measures at pre-program and following intervention completion (post-program), with an 8-week retention follow-up of quantitative assessments only. We hypothesized that exercise self-efficacy, exercise outcome measures, and moderative-vigorous leisure time physical activity (MVPA) behavior would significantly improve at post-program and benefits would be retained at retention. Congruence between numeric and thematic findings was present for exercise self-efficacy and self-evaluative exercise outcome expectations. Improved exercise self-efficacy was influenced by exercise knowledge gained during program participation. Increased expectations of internal exercise outcomes, such as influence on psychological state and overall mood, occurred following program participation. Themes on program-specific elements that facilitated increased intentional movement throughout daily life including portability and sustainability were not reflected in MVPA numeric assessment. This study is a critical first step toward systematically evaluating the effect of group tele-exercise on physical activity behavior for individuals with SCI and provides an important framework for the future evaluation of this program in randomized controlled trials. These findings demonstrate that this group tele-exercise program can positively influence critical SCT-driven personal determinants of physical activity behavior and has the potential to facilitate increased physical activity behavior.

Research Problem: Persons with spinal cord injury (SCI) face many barriers to physical activity (PA) participation. Research has demonstrated that participating in physical activity can have positive benefits for both quality of life (QOL) and reducing the risk of secondary health complications. Physical activity programs offered over the Internet have not been extensively researched, but may provide a solution to assist persons with SCI in overcoming barriers to PA participation. Methods: This integrated article dissertation focused on the feasibility of persons with SCI participating in a ten week exercise study delivered over the Internet to determine the impact of exercise on their quality of life and satisfaction with physical function, as well as their total number of PA participation minutes. The QOL outcome measures chosen for the study were selected based on a systematic review of QOL measures used in various PA interventions with persons with SCI. One additional aspect of this research was a comparison between a counseling group (online PA classes and four counselling sessions) and an active control group (online PA classes only) to determine differences in PA participation minutes and social cognitive predictors of PA participation. Results: With respect to participant satisfaction, it was determined that PA classes delivered over the Internet was a feasible delivery method. In each of the six domains of participant satisfaction, the median score was highly satisfied (4/4). The mean score for all participants also increased in each of the seven domains on the Satisfaction with Physical Function Survey from baseline to follow-up. On the Delighted/Terrible scale, 87% of the participants were 'pleased' or 'delighted' with the physical and psychological changes they perceived from participating in the intervention. In the counseling and control group study, there was a multivariate effect of group in the area of action planning. More specifically, the counseling group was better able to create action plans at all measurement time points compared to the control group. There were no statistically significant differences in total PA participation minutes between the two groups, nor were there differences in QOL over the length of the intervention. Conclusions: Physical activity programs offered over the Internet may be a feasible and acceptable delivery method for persons with SCI who may face barriers to PA participation. Participating in PA can result in positive perceptions of physical and psychological changes for persons with SCI.

There is overwhelming evidence linking increased physical activity with positive changes in cognitive functioning and brain health. Much of what we know about these interrelationships comes from aerobic exercise training studies with older adults and children. This literature has paved the way for the neuroscientific investigation of mechanisms responsible for exercise-induced cognitive and brain health enhancement, a list that ranges from molecular changes to systemic changes in executive control and neural connectivity. A new perspective has also emerged that aims to understand executive control processes that may underlie the regulation of health behavior. In accordance with this view, physical activity falls under the umbrella of health behaviors that require a substantial amount of executive control. Executive control is a limited resource, and the aging process depletes this resource. People who regularly exercise are said to have higher “self-regulatory control”—planning, goal-shielding and impulse control—than irregular exercisers. The successful maintenance of physical activity participation in lieu of daily cognitive stressors likely reflects an adaptive resistance to control failures. Indeed, a handful of studies have shown the relationship between greater executive control and subsequently higher levels of physical activity. However, little is known about the neural correlates of physical activity adherence or sedentary behavior, with the view that neurocognitive factors have an antecedent and reciprocal influence on these behaviors. No research has focused on the brain networks responsible for the self-regulation of physical activity, which likely overlaps with structures and functions playing critical roles in the regulation of other health behaviors. Interdisciplinary investigations are needed to explain the extent to which physical activity self-regulation and self-regulatory failure is dependent upon, or under the influence of executive control processes and brain networks. Understanding the degree to which self-regulatory resources may be enhanced, restored, and trained will have enormous implications for basic science and applied fields. It is also of great import to understand whether or not physical activity self-regulation is a domain-specific behavior associated with specific brain networks, or to determine the extent to which regulatory network-sharing occurs. The aim of this Frontiers Research Topic is to curate contributions from researchers in social and cognitive neurosciences and related fields, whose work involves the study of physical activity behavior, self-regulation and executive control. For this Research Topic, we, therefore, solicit reviews, original research articles, and opinion papers, which draw theoretical or empirical connections related to sustained physical activity behavior, self-regulatory strategies, cognitive performance, and brain structure and function. While focusing on work in the neurosciences, this Research Topic also welcomes contributions in the form of behavioral studies, psychophysiological investigations, and methodological innovations.
