This book reviews current knowledge of the etiopathogenesis, pathophysiology, and molecular biology of tympanic membrane retraction pockets and provides clear guidance on clinical assessment and treatment. A new diagnostic approach based on clinicoradiologic correlations is presented, and the ability of computer tomography to offer objective criteria for improved differentiation between stable and unstable or unsafe retraction pockets is explained. Surgical options are discussed, and on the basis of their own clinical and surgical experiences the authors propose a novel procedure, masto-atticotomy with anterior epitympanotomy (AER surgery), which addresses the causative factors underlying a retraction pocket. This surgery aims to restore adequate aeration routes for the middle ear compartments situated above the tympanic diaphragm; it results in better control of the pathology and in most cases prevents its recurrence. The book will assist otologists and surgeons in ensuring that retraction pockets are effectively diagnosed and treated, avoiding progression to cholesteatoma.

Dear colleagues, Promotion of the development of ENT is one of the most important tasks of IFOS (International Federation of Otorhinolaryngology). Apart from organizing the World Congresses it supports different international and regional meetings, organizes symposiums on well determined subjects, etc. Since the last World Congress in 1981, in addition to the Danube Symposium in 1982 and the Congress of ENT Societies in Asia Oceania in 1983, an international symposium was organized in Antwerp by the Belgian ENT Society on evaluation problems of middle ear surgery. This is a very controversial question on which there is no concensus. During a four-day meeting many anatomical and pathological aspects, operative methods, possibilities of the evaluation of the results with computers, were widely discussed. You will find all the lectures presented in this book. I am convinced that these papers will prove to be a good basis for further discussion and hope that finally we shall achieve an international agreement on the evaluation of our results in middle ear surgery. In preparing the material for this book, I took the advice of my students who generously shared their views and opinions with me. I was told that it would be preferable to have images of the various diseases with legends describing the disease. Students would be able to learn and retain the inf- mation more successfully if the material was accompanied by pictures and schematic drawings. Recent advances in technology have made it possible to photograph regions that are difcult to view with the naked eye, such as the ear, nose, throat, nasopharynx, and larynx – all the areas of otorhinolaryngology. Such an illustrated text in this feld is important and necessary for teaching purposes. In this book I tried to compile images of the basic conditions that are commonly seen in general practice and to give the reader a visual survey with a brief description of the condition. I added tables and schematic drawings in order to provide practical information. It is not the purpose of this book to be a comprehensive textbook, since many textbooks are already available with more detailed information of the conditions illustrated here. This book is primarily intended for medical students, family and general practitioners, and ENT trainees. It may also serve as basic reading material for those in allied specialties. I hope my colleagues fnd this book useful and it contributes toward their teaching purposes.
